Our work tackling multiple long-term conditions through addressing social risk factors
Social factors like meaningful work, financial health and housing are key to our strategy for two reasons: we believe they can profoundly affect people’s health and that they are not set in stone but are amenable to change.
Our work is to test how they can be addressed in ways that bring real impact to people’s lives, slowing down progression to multiple conditions. We also believe this will add to the much-needed evidence base on how to engage in the social determinants to practically improve people’s health.
