The impact of long-term health conditions is felt deeply in Lambeth and Southwark. Although more data and analysis are needed to understand the distribution of multiple long-term conditions here, the numbers are high, with an estimated 176,000 (more than one in five) residents living with at least one long-term condition and over 19,000 with three or more. This especially stands out given Lambeth and Southwark have much younger populations than the London average.
Analysis suggests that a complex mix of factors are resulting in earlier onset, particularly in deprived communities. Many of these drivers are societal and a product of the environment in which people live and the lives they lead.
Through our recent Southwark and Lambeth Integrated Care (SLIC) project, we have learned that people with long-term conditions consistently say that:
Yet, people living with one or several long-term conditions often have a poor experience of care, with challenges including how to prioritise treatment for their conditions, adhering to multiple medications, fragmented care and even conflicting advice.
