Since launching our programmes in March 2017, we have spent time deepening our understanding of the evidence base and our local reality.
We have learnt that, in deprived and diverse areas like our London boroughs of Lambeth and Southwark, living with more than one chronic health problem is becoming increasingly common. For every five people who have diabetes, four also have at least one other chronic condition.
We are now exploring and testing assumptions through a small number of learning partnerships and projects.
We are now in the development phase of our programme. During this phase we aim to:
To help us with this we have asked ourselves some key questions, and have established a small number of partnerships to help us find answers. We will use what we learn during this phase to inform the next stages of our programme, and share what we learn with others also interested in multiple long-term conditions and health in urban areas similar to our boroughs.
